- Initial Consultation with Dr. Green.
He will be asking you about your symptoms (areas of pain), concerns, history (family, health, past medical history, injury) current medications (if any), and lifestyle, etc.
- Examination
During your initial exam, Dr. Green will check your vital signs (blood pressure/pulse/respiration), messure your neck and back range of motion, and
perform a orthopedic/neurological evaluation. These tests will allow him to properly evaluate your condition.
- X-Rays
Dr. Green will review your films in x-rays to determine any issues with your spine, hips, neck, extremities (knees, hands, etc.), which may be the cause of your pain or discomfort. You wil be able to see your x-rays; Dr. Green will show you any misalignments, bone spurs, disc dengeneration or other disc diseases, etc.
(If you have X-Rays taken within the past 12 months, please bring them with you).
- Report of Finding
Once Dr. Green has reviewed you history, symptoms and x-rays, he will go over his suggested treatment plan with you.

Please allow 10-15 minutes before your appointment for 1st forms
- X-Rays and/or any recent MRI reports.
- Insurance Information - We will assist you with benefit verification during your first visit.
- Call your insurance company using the 800 member number on the back of
your insurance card.
Ask:
- the representative to verify your Chiropractic benefits
- if you have any Physical Therapy benefits
- if you have a deductible and the remaining deductible amount, if any, and if the deductible applies to chiropractic or physical therapy.
- what your visit limit is (if any), or maximum dollar amount for Chiropractic Care
- ask if you have a copay coinsurance for both chiropractic and physical therapy
- Massage Benefits: If Dr. Green recommends deep tissue or triggerpoint therapy you will need to ask specifically if you have massage therapy coverage. Ask the representative to check the massage billing CPT code: 97124
- Acupuncture, Spinal Decompression, Therapeutic Exercise, and
Nutrition Counseling also have special codes. If you are interested in any of these services please ask the member representative to verify benefits.

The Point of Service discount is best when:

Dr. Green is a expert witness for auto accidents and a accident reconstructionist. He has experience treating severe whiplash injuries, as well as many other musculo-skeletal injuries caused by your accident.
If you have had a recent accident, please bring your accident report, your PI lawyers contact information (if you have a lawyer representing you), your auto medical pay information, and any other information that may assist Dr. Green with your case, including any recent MRI's or Xrays.
We will work with your auto med pay, PI attorney and/or your claims adjustor in order to assist you with paying your medical bills.
